Title:	General Application: RN/LPN (apply online)
Description:	Responsible to provide extended hour (full 8-12 hr shifts) of home care for children and young adults who have special medical problems. Identify and meet physical and emotional needs of clients (including clients and clients family or designated members of the clients family. Utilize and implement Caregivers Network policies and procedures as established by Caregivers Network nursing administration in the planning and delivering of total client care. Contribute to the development of a comprehensive educational plan specific and appropriate to each client/family. Communicates in a clear and effective manner with client/family, other nursing staff members, Caregivers Network administration, attending physicians and other members of the health care team. Promotes positive professional client/family- Caregivers Network relationships. Assesses and maintain a safe and nurturing environment for the client/ family. Display professionalism in the client’s home, school, clinic visits, etc. Participate in ongoing and special performance improvement activities and relevant education to improve nursing practice.

Qualifications:	Graduate of an approved school of professional nursing. Active license as an RN from the MN and/or WI board of Nursing. Reliable transportation and valid Minnesota driver’s license.
